Student Exchange in Madrid – June 2018 Dates:
Sunday, June 10, 2018 – Meet at Delta Check-In at 9:15 am
Tuesday, June 26, 2018 late evening return.

Hosting in Grand Rapids – July 2018 Dates:
Tuesday, July 3, 2018 – arriving at 7:58 pm.
Sunday, July 22, 2018 – be at airport at 1:00 pm

Meeting Dates for 2018 Exchange.
Unless advised otherwise, meetings start at 7:30 pm and end at approximately 9:00 pm at the Image Building (2828 Kraft Ave SE)
The participating student and at least one parent
are required to attend all meetings:

Deadlines:
By Thursday, December 7, 2017:
Student Questionnaire – must be completed by Student
Student Profile Form- must be completed by the student
Student Information & Essay – must be completed by the student
Parent Questionnaire – must be completed by parent
Parent Nomination Form  – completed by a legal guardian
Host Family Information – completed by a legal guardian

By Monday, February12, 2018:
Trip Agreement – signed by student and all legal guardians
Behavior Standards Contract – signed by student & a legal guardian
$775.00 deposit (includes the $175 insurance premium)
made payable to Image Tours

By Monday, April 16, 2018:
Health Form  – completed by a legal guardian
Homestay Referral Form – completed by a family friend
$2000 payment check made payable to Image Tours
3 copies of participating student’s passport name page
(or copy of the passport application)
Traveling Minor Parent Consent Letter
(this will need to be notarized for all legal guardians)
