2018 U.S. Visit Coordinator Instructions

The coordinator for each U.S. excursion/function needs to complete the following form  and submit it to the U.S. Coordinator and Monique Kasmauskis by the April 16th, 2018 Meeting.                            Resubmit this form with any changes prior to the May 31st meeting.

Complete the following form and submit to Monique Kasmauskis to request prepayment or reimbursement.
All payment requests must be accompanied by an invoice or receipt.
Prepayment requests must be turned in prior to May 24th, 2018.  Reimbursement requests must be turned in prior to Aug. 16th 2018.

It is a good idea to print an extra copy for your personal files and to save to your desktop before emailing or closing each form.
